The Hidden Hierarchies Inside Hospitals

Hospitals look orderly from the outside, but inside, they’re ecosystems with unspoken rules.

Some of the hierarchies I weave into my fiction:

• Doctors vs. administration

• Nurses vs. policy

• Specialists vs. generalists

• New staff vs. veterans

• Day shift vs. night shift

• Those who follow the rules vs. those who bend them

These dynamics create natural conflict — and conflict drives story.

When a crisis hits, these hierarchies either:

• Snap into place or

• Completely collapse

Both outcomes make for incredible storytelling.
